Your Rights

You may:

Inspect or receive a copy of your medical record (electronic or paper). We will provide it or a summary within 30 days of your request and may charge a reasonable, cost‑based fee.
Request an amendment to information you believe is incorrect or incomplete. We will respond in writing within 60 days; if we deny the request, we will explain why.
Ask for confidential communications (e.g., only at a specific phone number or address). We accommodate all reasonable requests.
Limit what we use or share for treatment, payment, or operations. While we are not required to agree to every limitation, we will honor requests to withhold information from an insurer when you pay for the item or service in full out‑of‑pocket, unless sharing is required by law.
Receive an accounting of disclosures made during the six years preceding your request (one free list per year).
Obtain a paper copy of this notice at any time, even if you previously accepted an electronic copy.
▶ Appoint someone to act for you (e.g., a medical power of attorney or legal guardian). We will confirm that person’s authority before acting.
▶ File a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights if you believe your rights have been violated. We will not retaliate against you.

Your Choices

In certain situations you may direct us to:

Share information with family, friends, or disaster‑relief organizations involved in your care.

Include your information in a facility directory.

Contact you for fundraising.

If you are unable to express a preference (for example, if you are unconscious), we may share information if we believe it is in your best interest or to lessen a serious, imminent threat to health or safety.

We never use or disclose your information for marketing, sell your information, or share psychotherapy notes without your written permission. You can tell us not to contact you again for fundraising.

Our Uses and Disclosures

We typically use or share your PHI to:

Treat you – e.g., a pharmacist talks with your doctor about medication interactions.

Run our organization – e.g., quality assessment, staff training, or contacting you about refills.

Bill for services – e.g., submit claims to your health plan for payment.

We are also allowed or required to share PHI for public‑health and safety purposes (disease reporting, recalls, abuse reporting), research, compliance with the law, organ‑donation, coroner or funeral‑director duties, workers’ compensation, law‑enforcement activities, national security, and in response to court orders or subpoenas, subject to the conditions of federal and state law.

Our Responsibilities

We must maintain the privacy and security of your PHI and notify you promptly if a breach occurs that may have compromised it.

We follow the duties and practices described in this notice and will not use or disclose your information beyond what is described here unless you give written permission. You may revoke that permission at any time.

2024 HIPAA Final Rule: Recent federal regulations strengthen protections for PHI related to lawful reproductive‑health services and Substance Use Disorder records. Covered entities must update their Notices of Privacy Practices by February 16 2026 to include required language.

Changes to This Notice

We may change the terms of this notice at any time. Changes will apply to all PHI we maintain. The revised notice will be available in our pharmacy and on our website.
